Our Community Care Team provides care to people who are too ill or frail to come to the surgery. They work between 8am and 5pm, seven days a week.
They are involved with patients discharged from hospital requiring follow up nursing care at home and patients suffering from chronic or terminal conditions.

The team will provide the care that is most appropriate according to your needs which may be met by one or more of the professionals within the team. The nurses can obtain specialised equipment, including continence products, to help with daily living.
