If you cannot register online, then please print a copy of the registration form below and a questionnaire below and return these to the surgery with some form of ID as stated below.

When returned, you should provide proof of ID, one form of ID is required, ideally your Passport or Driving Licence.  Other documents, such as Library Cards, Video Rental Cards, Health Club Cards and Private Rent Books, are not acceptable.

Please also let us have your repeat prescription list of any medication or the tablet bottles/packs themselves. If you have a complicated medical history please ask your old GP for a summary printout of your old records as it can take a few months for the surgery to obtain your old records.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
